Yavapai College is a participant in the Yavapai Combined Trust (YCT) referred to as the “Trust.” The Yavapai Combined Trust is comprised of four entities – Yavapai College; Town of Chino Valley, City of Prescott and Yavapai County.

The Plan is self-insured plan with claims paid by independent claims administrators and the Plan is not subject to the provisions of the Employment Retirement Income Security Act of 1974 (ERISA).

The Trust provides coverage and administration for Medical (includes prescription coverage), Dental, Vision, Life Insurance, Wellness Program, Flexible Spending Account Plans, Short Term Disability, the Employee Assistance Program and COBRA.

All claims for Medical, Dental, Vision, Flexible Spending Accounts, Short Term Disability and COBRA are paid by the independent third party administrator “Administrative Enterprises, Inc. (AEI).”

When choosing a health insurance plan, you are also choosing the plan's approach to the provision of benefits and payment for health care services.

  • Compare premiums and benefits to predict costs
  • Premiums are deducted each payday, whether or not services are used
  • Co-payments, deductibles or coinsurance fees may be charged when services are used depending on plan selected and type of service
  • Review all plan materials to understand plan terms and conditions of use

While recognizing the many benefits associated with this Plan, it is also important to note that not every expense you incur for health care is covered by the Plan.

Yavapai Combined Trust intends this Plan to be permanent, but since future conditions possibly affecting the Plan cannot be anticipated or foreseen, the Trust reserves the right to amend, modify or terminate this Plan at any time, which may result in the termination or modification of your coverage. Expenses incurred prior to the Plan termination will be paid as provided under the terms of this Plan prior to its termination.

IT IS YOUR RESPONSIBILITY TO UNDERSTAND YOUR COVERAGE

  • Choices that you make, or that are made on account of a referral by your
  • physician, that result in out-of-network charges or medically unnecessary care that is not payable by the Plan are YOUR responsibility.
  • A referral from an in-network physician to an out-of-network physician does NOT make the claim from the out-of-network physician payable at the in-network rate.
  • Your Human Resources office and the Plan’s Claims Administrator are available to help answer questions and to explore options for coverage, but ultimately it is your responsibility to understand this Plan.

IMPORTANT NOTICE

You or your Dependents must promptly furnish to the Plan Administrator, within 30 days of the change, information regarding change of name, address, marriage, divorce or legal separation, death of any covered family member, change in status of a Dependent Child, Medicare enrollment or disenrollment or the existence of other coverage.

Failure to do so may cause you or your Dependents to lose certain rights and/or coverage under the Plan.
