For your first order with the mail service pharmacy, simply complete the registration form enclosed in your Yavapai Combined Trust prescription drug program packet. You will need to send a new prescription along with the enclosed registration form to the mail service pharmacy.
If you feel that you will run out of medication while you are submitting your prescriptions to the mail service pharmacy, please ask your doctor to write you two prescriptions. One for a 30 day supply that you can fill at a local pharmacy right away, and the other for a 90 day supply that you can mail in to the mail service pharmacy with your completed registration form.
To find out which pharmacy is participating under your pharmacy benefit plan, you may call the WHI Member Services department, 24 hours, 7 days a week at 1-800-207-2568 or find the nearest participating pharmacy via the WHI website at www.mywhi.com.
There are currently over 54,000 pharmacies nationwide that are in your network. Even though Walgreens is a participating pharmacy under your plan, please note that you are not required to only use a Walgreens pharmacy to fill your retail prescriptions.
