Staff employees will receive payment for authorized hours on the next regularly scheduled pay date.
Payment for any unused qualified leave hours will be paid on a separate check on that pay date.
Employees who retire after 15 years of continuous service and who are approved for retirement will be paid for accumulated unused sick leave, up to 70 days, at the rate of $60 per day, not to exceed $4200.
Your final check (s) may be auto-deposited if you are set up for that service, or you may request a live check.

You will continue to receive full medical/dental/vision benefits from Yavapai Combined Trust (YCT) through the end of the month in which your employment ends.
All medical/dental/vision and prescription claims must be submitted to YCT within 12 months of the date of service. No YCT Plan benefits will be paid for any claim not submitted within this period.
If applicable, you may seek reimbursement under the Medical Flexible Spending Account for expenses incurred through the last day of the month in which you were employed; you may seek reimbursement for Dependent Care Flexible Spending Account through the end of the Plan Year.
Information regarding COBRA (continuing health care coverage at the current coverage level, at your expense) will be mailed to your home within 10 days of your separation from Administrative Enterprises, Inc. (AEI).
AEI will send you COBRA information to the address on their record; the information will include the enrollment application and premium costs. If you wish to elect COBRA, you will need to complete the enrollment application and return to AEI along with the applicable premium costs. Once AEI receives the premium amount, this will activate your COBRA coverage.

Employees retiring after 15 years of continuous service will receive up to a maximum of $1,000 per year for premiums toward the retired employee’s health insurance up to age 65 years. This is prorated and paid out quarterly (April, July, October, and January).
Computer loan, if applicable, is due and payable upon separation. Any outstanding balance will be withheld from the final check, and if additional payment is required you will be asked to present that amount upon termination.
Retirement contributions, including any Supplemental Retirement Plans 403(b) and 457 will be withheld from your final regular paycheck. Information regarding cash ability, transferability, and other options can be obtained by contacting the providers at the numbers listed below:

For information regarding your life insurance (Basic Life Insurance and Voluntary Life Insurance); and you wish to continue to be covered for life insurance you had at time of termination. You may be eligible to either “Port” or “Convert” your coverage.

For information regarding your Cancer policy, you will need to call the Allstate Insurance carrier at (800) 521-3535.
If you (or a dependent) are enrolled in a class through tuition waiver, you may continue through the end of the semester.
