Citizenship & Immigration Documents

To comply with Proposition 300 Yavapai College uses state and federal laws and regulations to designate forms of documentation that serve as evidence of citizenship, permanent residence or lawful presence. A student must present original documents or legible copies to the College. The preferred method is to submit documents in person to the Student Enrollment & Advising Center at these College sites:

  • Prescott: 1100 E Sheldon Street
  • Verde Valley: 601 Black Hills Drive, Clarkdale
  • Prescott Valley: 6955 Panther Path
  • Chino Valley: 2275 Old Home Manor Drive

The College will accept legible photocopies (with no evidence of tampering) sent by fax to (928) 777-3135 or mailed to: Yavapai College Student Enrollment & Advising Center, 1100 E. Sheldon St., Prescott, AZ 86301.

1. Certify Citizenship

The U.S. Department of Education will automatically certify citizenship of students with an approved FAFSA (Free Application for Federal Student Aid).

2. Designated Primary Documents

Designated primary documents - Any one document from the following list may serve as evidence of citizenship or lawful presence in the United States.

  • Unexpired Arizona driver's license, instruction permit or non-operator ID issued on or after October 1, 1996
  • Birth certificate verifying citizenship to the satisfaction of the Registrar (U.S. state, District of Columbia, Puerto Rico, U.S. Virgin Islands, Guam, American Samoa, Swain's Island, Northern Mariana Islands, other U.S. territory or possession)
  • passport, current or expired
  • U.S. military DD-214
  • U.S.military ID card (active duty, reserve or retired)
  • U.S.permanent resident or resident alien card (Form I-551)
  • U.S.temporary resident card (Form I-688)
  • U.S.certificate of naturalization (N-550 or N-570)
  • U.S.certificate of citizenship (N-560 or N-561)
  • BIA or tribal identification card
  • BIA or tribal affidavit of birth
  • Tribal certificate of Indian blood
  • U.S. State Department certificate of birth abroad (FS-240, FS-545, or DS-1350)
  • Foreign passport with U.S. visa
  • USCIS I-94 or I-94A form (arrival/departure record)
  • U.S.alien registration receipt card (Form I-151)
  • U.S.travel document endorsed "permit to re-enter Form I-327"
  • U.S.refugee travel document (Form I-571)
  • U.S.application for voluntary departure (family unity program Form I-797)
  • U.S.employment authorization card (Form I-688A, I-688B or I-766)
  • U.S.notice of approval to apply for permanent residency (Form I-171 or I-464)
  • Other documentation issued by U.S. Citizenship and Immigration Services or representing a status permitted under 8 U.S. Code sections 1621 and 1641

3. Designated Secondary Documents

Designated secondary documents - In lieu of a primary document from the list at left, the student may provide a legible photocopy (with no evidence of tampering) of two (2) of the following, provided that (1) one is a photo ID and (2) one shows date of birth.

  • Unexpired Arizona driver's license, instruction permit or non-operator ID issued before October 1, 1996
  • Driver's license, instruction permit, or non-operator ID issued by another U.S. state or possession
  • Social security card
  • W-2 form
  • Bank card
  • Credit card
  • Employee ID badge (with photo)
  • School ID (with photo)
  • Medical insurance ID card (including AHCCCS ID card)
  • U.S.military dependent ID card
  • U.S.armed forces driver's license
  • Motor vehicle record or clearance letter (within 30 days of issue)
  • Legal guardian affidavit
  • Selective service card
  • U.S. Department of Veterans Affairs card
  • Concealed weapons permit
  • Professional license
  • Marriage certificate issued in any U.S. state, territory or possession
  • Affidavit of identification (with photo and within 15 days of issue) from Arizona Department of Corrections
  • Released offender identification from Arizona Department of Corrections
  • Certified letter of identification for a ward of the court, issued by a court or government agency in the U.S.
  • Certified copy from court of record of divorce decree, adoption decree, name change decree, bankruptcy decree, or emancipation decree.
  • Other documentation that reasonably establishes that the individual is a U.S. citizen, permanent resident, or has lawful immigration status.
