Ways to report an incident at Yavapai College

Use the following forms to report incidents related to Yavapai College.


  • Incident Report - Anyone within or outside the Yavapai College community may use this report
  • Student Academic Appeal - A student may appeal an academic or instructional decision by faculty. The appeal must be made in a timely manner in accordance with established procedures and criteria defined in Policy 3.16. Please review the policy prior to submitting the appeal.
  • Care Reporting Form - Use this form to report people you feel may be a threat to themselves or others. A CARE team member may also be reached by calling 928-776- 2273. If the threat is immediately life threatening call 911.
  • Sexual Misconduct Report - This form should be used to report concerns pertaining to possible violations of Yavapai College's Sexual Misconduct Policy such as dating violence, domestic violence, hostile environment, sexual assault, sexual exploitation, sexual harassment, stalking, retaliation, or other forms of sexual misconduct.
  • Academic Integrity Report - For instructors
  • Smoking Reporting Form
