Incident Reporting

Use the following forms to report incidents related to Yavapai College.


  • Conduct Incident Reporting Form - Anyone within or outside the Yavapai College community may use this report
  • CARE Reporting Form - Use this form for individuals who need mental health support or basic needs support services. For immediate life-threatening concerns about someone who is a threat to themselves or to others, please call 911 and Campus Safety at 928-237-0420.
  • Sexual Harassment Reporting Form - This form should be used to report concerns pertaining to dating violence, domestic violence, hostile environment, sexual assault, sexual exploitation, sexual harassment, stalking, retaliation, or other forms of sexual misconduct.
  • Discrimination Reporting Form - This form should be used to report concerns pertaining to discrimination on the basis of race, color, national origin, age, sex, sexual orientation, citizenship status, veteran status, gender identity, religion, disability or any legally protected classification.

  • Academic Integrity Reporting Form - For instructors
  • Academic Complaint - Student Academic Appeal Form - A student may appeal an academic or instructional decision by faculty. The appeal must be made in a timely manner in accordance with established procedures and criteria defined in Policy 3.16. Please review the policy prior to submitting the appeal.
