Report behavior or an incident that may involve a violation of Yavapai College expectations, policies, or standards of conduct.
Anyone within or outside the Yavapai College community may submit this report.

Use these forms to report conduct concerns, request support for someone, document an injury, or begin an academic appeal. You do not need to be certain which policy applies before sharing a concern.
These forms may be used by students, employees, visitors, or members of the wider community, depending on the situation.
Report behavior or an incident that may involve a violation of Yavapai College expectations, policies, or standards of conduct.
Anyone within or outside the Yavapai College community may submit this report.
Share a concern about someone who may need mental health assistance, basic-needs support, or outreach from the college.
A CARE report may be submitted when you are worried about a person, even when you do not know exactly what support they need.
Report dating violence, domestic violence, hostile-environment concerns, sexual assault, sexual exploitation, sexual harassment, stalking, retaliation, or another form of sexual misconduct.
You may submit a concern about something that happened to you, something you witnessed, or information shared with you.
Report discrimination based on race, color, national origin, age, sex, sexual orientation, citizenship status, veteran status, gender identity, religion, disability, or another legally protected classification.
Use these forms for injuries occurring at Yavapai College, academic integrity concerns, or appeals of academic and instructional decisions.
Report an injury incurred at Yavapai College, even when medical treatment was not used.
Deadline: Complete the report within 24 hours of the injury.

Instructors may use this form to report an academic integrity concern involving a student or course.
A student may appeal an academic or instructional decision made by faculty. Appeals must be submitted within the established timeline and meet the criteria defined in college policy.
